When Should You Consider BPH Surgery?

Signs It's Time to Stop Waiting

BPH is rarely an emergency, and most men can live with mild symptoms for years. But there's a tipping point, a moment when "managing" with pills, scheduling life around bathroom access, and waking up multiple times a night stops being acceptable. Knowing where that line is for you is the key question.

1. Medications Aren't Doing Enough Anymore

Alpha-blockers (tamsulosin, alfuzosin) and 5-alpha reductase inhibitors (finasteride, dutasteride) work well for many men, but not forever, and not for everyone. If your symptoms are creeping back despite maximum medical therapy, surgery is a reasonable conversation to have.

2. The Side Effects of Medication Aren't Worth It

Dizziness, fatigue, decreased libido, ejaculatory changes, and nasal congestion are common reasons men want off BPH medications. A one-time procedure like Aquablation can eliminate the need for these drugs entirely in most cases.

3. Your IPSS Score Is Climbing

The International Prostate Symptom Score (IPSS) is the standard way urologists quantify BPH severity:

  • 0–7: Mild
  • 8–19: Moderate
  • 20–35: Severe

Scores above 19, or moderate scores with high quality-of-life impact, are typical thresholds for surgical evaluation.

4. You've Developed Complications

These are stronger indications for surgery, sometimes urgent:

  • Acute urinary retention (inability to urinate)
  • Recurrent urinary tract infections
  • Bladder stones
  • Visible blood in the urine attributed to BPH
  • Kidney function changes from backup pressure

5. Your Quality of Life Is Suffering

This one isn't on a lab report. If you're avoiding road trips, declining social plans, sleeping poorly because of nighttime urination, or simply tired of organizing your life around the nearest bathroom, that's a legitimate reason to act. Quality of life is a clinical metric, not a luxury.

What Comes Next

If any of the above resonates, the next step is a consultation that includes a symptom score, uroflow testing, a post-void residual, and an honest discussion of your treatment options, including Aquablation. Most men who finally schedule the consultation say the same thing afterward: I should have done this sooner.
